Supply of commercially available Iodine 131 by Medi Radio Malaysia Sdn Bhd.

With reference to the above, we hereby confirm the following:

1. Registered I131 Products

At the time of writing and to the best of our knowledge; the following are the only registered Iodine-131 products commercially available in Malaysia:

·Iodosol Sodium iodide (131I) 37-740 MBq/ml solution for injection, MAL23126015AZ

·Iodopol, 37-7400 MBq, hard capsules, MAL23126014AZ 

In the event of supply interruptions, Medi Radio Malaysia Sdn Bhd can procure alternative products, subject to the hospital providing a valid import permit exemption.

2. Quotation Validity

   The quotations titled “MRM Quote I131 Solution Peninsular Malaysia 2025_V02” & “ MRM Quote I131 Capsules Peninsular Malaysia 2025_V02” is valid till 31^st December 2025.
   

3.   Calibration & Order Lead Time All calibrated activities are scheduled for Mondays only.  To ensure timely processing, orders must be submitted by 2:00 PM on Monday, at least four (4) weeks in advance of the intended calibration date. For cases requiring treatment outside of the standard Monday calibration, the document titled “I131_Capsules_Decay_Table_V01” will be provided to facilitate accurate ordering and dosage planning.

4.       Order Confirmation Orders are confirmed only upon receipt of Purchase Order sent via email to sales@mrmpharma.com

5.       Purchase Order Requirements The Purchase Order must include:

·         Name and address of the purchaser

·         Date of order

·         Name and quantity of the poison

·         Calibration date

·         Purpose

·         Purchaser’s signature

6.       Payment terms All payments are due 30 days from the date of delivery and invoice issuance. Kindly send proof of payment to finance@mrmpharma.com

7.       Late Payment Charges A late payment charge of 1.5% per month will be applied to any outstanding balance beyond the stated due date. This charge will be calculated daily until full payment is received
